Ramp middle is hinged to lift up. I was able alone to tilt ramp on a corner to fold it completely (without picking it off the ground), then put on a cart to move it.

When used on steps, does the bottom of the ramp fold up toward the top? Does ramp have to be totally picked up to fold?

This ramp does fold in half, so the bottom would fold toward the top. The ramp does need to be picked up or flipped over to fold since the hinges are on the underside of the ramp.

If I don't need the ramp to fold, can I use one solid piece of plywood. It is fine if I need the hinges installed to make the rails work- they just wouldn't actually do anything in this scenario.

Using a single sheet of plywood would require modifications to the ramp kit and would not be recommended.

While you could place two ramps side by side, the ramp kit is not designed to be able to attach the ramps together for a wider ramp.

Other parts needed?
Is there anything else that should be added to this kit other than the plywood? Does it fold completely into the trailer? I noticed the red handles at the door

Only plywood needs to be added to this unit. Folded in half, the unit is 3’ in length and should fit inside most trailers and flatbeds.

Can this be used to load a pickup truck?
I am confused about how fixed the "height" of this ramp is. Can I use it to load lawn equipment in to a pickup truck?

This ramp can only handle a 3" rise per foot. With this ramp measuring 6 feet the maximum rise this lift can handle is 18". It is not designed to load safely at steeper angles Northern tool has loading ramps for pickups. See below.
